Many high-income professionals follow the traditional playbook: work harder, earn more, invest in the stock market, and hope it all works out. In this episode of the Next Level Income Podcast, I sat down with Dr. Tyson Cobb—semi-retired orthopedic surgeon, investor, and author of Wealth by Design—to challenge that narrative.
Dr. Cobb spent decades earning a strong income, yet like many doctors and professionals, he found himself overworked, heavily taxed, and exposed to market volatility. Everything changed when he began shifting away from stocks and into real estate and alternative investments. What surprised him most wasn’t just better returns—it was regaining control of his time and lifestyle.
His first real estate deal, a triple-net lease, showed him the power of leverage, depreciation, and predictable cash flow. As he put it, it almost felt “too easy.” That realization launched a deeper journey into syndications, business investing, and eventually teaching others how to design wealth intentionally.
The BONES Framework
Dr. Cobb outlines his philosophy using the BONES Framework, built specifically for high-income professionals:
Believe – Wealth starts with mindset. If you can master a demanding profession, you can understand how money works well enough to make informed decisions.
Organize Your Network – The right relationships shorten the learning curve. Your network truly is your net worth.
Nurture Your Knowledge – You may not have free time, but you have available time. Podcasts, audiobooks, and learning on the go add up.
Embark (Take Action) – You’ll never feel fully ready. Prepare wisely, then take the first step—even if it’s uncomfortable.
Serve – True wealth includes giving back. Serving others creates meaning, abundance, and long-term fulfillment.
Opportunity in Uncertain Markets
We also discussed today’s investing environment. Multifamily real estate has been challenged by higher interest rates and forced sellers, but history shows that periods of fear often create the best long-term opportunities. Dr. Cobb is also excited about roll-up strategies and business acquisitions, particularly as baby boomers retire and sell profitable companies.
Final Thoughts
Financial freedom isn’t about retiring early or chasing returns—it’s about designing a life with more time, flexibility, and purpose. As Dr. Cobb’s journey shows, wealth doesn’t happen by accident. It’s built intentionally.
